Gwendolyn Hail is the Transition of Care Manager at Northeast Alabama Regional Medical Center. She is speaking about the Re-Admission Measure and how penalties are assessed for Acute Care Hospitals. With a background in nursing as a Licensed Practical Nurse, she has worked in several healthcare settings to include Skilled Nursing Facilities, Home Health, Hospice, and Private MD Practices over the last 25 years. Gwendolyn was originally employed at Northeast Alabama Medical Center as a Transition of Care Analyst but has grown into the Manager position as the Transition of Care department has evolved into a Post-Acute Navigation Platform for High Risk Populations. She is passionate about leading the navigation team at her hospital and assisting the geriatric patient populations recover successfully after a hospital stay. The Transition of Care team at Northeast Alabama Regional Medical Center has helped to reduce Re-Admission penalty dollars and improve patient care and outcomes over the past 5 years.
