WHIMA | Common Missed Physician Query Opportunities and Key Indicators to Look for in the Chart
Overview
A query can be a powerful communication tool used to clarify documentation in a health record and achieve accurate code assignment. Querying the physician has become a common communication and educational tool for coding departments. An effective query process aids the hospital’s compliance with coding rules and serves as an educational tool for providers, and coding professionals, on the physician side. This session will include Common Missed Physician Query Opportunities and Key Indicators to look for in the chart and also when to query and when not to query.
Objectives:
- Review clinical indicators and coding guidelines for Sepsis. Provide case studies on how and when to query for sepsis.
- Review clinical indicators and coding guidelines for cardiac complications such as elevated troponin, type 2 MI, and type I MI. Provide case studies on how and when to query for elevated troponin and myocardial infarction.
- Review clinical indicators and coding guidelines for respiratory complications
- Review clinical indicators and coding guidelines for renal complications
- Review clinical indicators and coding guidelines for common neurology complications such as Altered Mental Status, Encephalopathy, Delirium, and Dementia. Provide case studies on how and when to query for altered mental status.
