Introduce learners to Risk Adjustment principles and guidelines with a focus on the Medicare HCC model. Review examples of complete and incomplete documentation and coding and how incomplete coding can impact providers and payers.  Explain the benefits of Risk Adjustment from a patient, provider, and payer perspective.  

Review coding trouble spots in the medical record – problem lists, past medical history and conditions documented as “history of”.  Share documentation best practices to reduce unclear documentation.
