Overview

Clinical Documentation Integrity Programs, formally known as Clinical Documentation Programs, started in Fiscal Year 2008 with the introduction of the MS-DRG system. This revision was designed to better reflect and adjust for patient severity of illness. Since this introduction, CDI programs have continued to evolve, branch out, and incorporate more technology to enhance overall efficiency in processes to drive revenue for the hospital. There are a myriad of CDI software solutions available in the marketplace utilizing different methodologies to assist CDI specialists in reviewing records, managing chart reviews, and helping identifying opportunities for CDI intervention in the name of CC/MCC capture, clarification of diagnoses, capture of quality measures, and severity of illness/risk of mortality measures. Over the last eight years, there has been an unrelenting focus in CDI programs as a whole upon reimbursement through use of CDI software that is intended to enhance efficiency of chart review and CDI impact upon reimbursement and quality measures. The CDI profession has deviated and drifted away from the original intent and purpose of improving quality and completeness of documentation and communication of patient care. In 2019 the OIG announced in its Work Plan of its intention to validate its concern of upcoding- the practice of mis over-coding. In February of 2021 the OIG released a report titled Trend Toward More Expensive Inpatient Hospital Stays in Medicare Emerged Before COVID-19 and Warrants Further Scrutiny. This report validated the OIG’s concern of upcoding and showed the following: Hospitals are increasingly billing for inpatient stays at the highest severity level, which is the most expensive one. The number of stays at the highest severity level increased almost 20 percent from FY 2014 through FY 2019, ultimately accounting for nearly half of all Medicare spending on inpatient hospital stays. The number of stays billed at each of the other severity levels decreased. At the same time, the average length of stay decreased for stays at the highest severity level, while the average length of all stays remained largely the same.
