Hospital accreditation establishes a foundation for quality and patient safety, but achieving measurable improvement requires intentional action beyond compliance. This session connects research with practical application. Using findings from a comparative study of The Joint Commission and DNV-accredited hospitals, presenters will examine how accreditation relates to CMS patient safety outcome measures. Building on these results, the presenters will share how their organization implemented targeted interventions to enhance sepsis outcomes outside of their regulatory framework—demonstrating how evidence-based initiatives can bridge the gap between accreditation standards and sustained quality performance.

This presentation will take listeners through my journey from pre-diagnosis with a rare liver disease called Primary Sclerosing Cholangitis to a life saving liver transplant. I am now 10 years post-transplant.

As healthcare organizations merge, person/patient identity management becomes increasingly complex. We will examine the pivotal role of Health Information Professionals (HIPs) in data conversion planning; key pre-conversion steps will be outlined, including extract specifications, record retention considerations, and testing strategies. The presentation will address data analysis, duplicate record rates, and effective patient matching/linking strategies. This session reveals the risks of over-relying on AI and explores how human oversight enhances patient matching accuracy. Attendees will experience real-world challenges through interactive exercises and gain insights into best practices for data integrity, duplicate resolution, and compliance in M&A transitions.
