This presentation will share the story of the implementation and growth of the Winona Community HUB in a rural community in Minnesota from two different perspectives. The Winona Community HUB is a certified Pathways Community HUB (PCH), which is an evidence-based and community-based model of care coordination across social and health services that provides support at the household-level and offers a closed-loop referral method of tracking risk mitigation. PCH HUBs deliver services through a network of community care agencies employing Community Health Workers and are supported through an outcomes-based reimbursement model braided with other funding streams. Presenters will also share how the Winona Community HUB collaborates across community care agencies to provide comprehensive training and other supports to sustain the entire network.

Learning Objectives:

  1. Attendees will be able to describe the basic structure of the Pathways Community HUB model
  2. Attendees will gain insight into the roles of both the administrators of the HUB as well as the community care agencies employing HUB Community Health Workers
  3. Attendees will understand the financial and administrative support structures that sustain the HUB itself as well as its partner community care agencies.
